Marking a decade since last polio case: WHO SEAR countries gear up for massive vaccination campaign – this time for COVID-19 virus

New Delhi, 13 January 2021 – On this day in 2011, WHO South-East Asia Region reported its last case of wild poliovirus from West Bengal in India. A decade later, countries in the Region are gearing up for massive vaccination campaigns, taking lessons from polio program, but surpassing its scale and size in a bid to end COVID-19 pandemic.

“We are witnessing unprecedented efforts by Member countries to protect their vulnerable population against COVID-19 with vaccines,” said Dr Poonam Khetrapal Singh, Regional Director, WHO South-East Asia.

Indonesia rolled out COVID-19 vaccination today, while preparations are in full swing in India for one of the world’s biggest vaccination programme beginning 16 January, and in other countries for their campaign in the coming months.

Safe and effective vaccines can be a gamechanger if accessible across the world and to all vulnerable population within the countries.

Vaccines will help to curtail the COVID-19 pandemic. However, they won’t solve everything by themselves as initially they are bound to be in limited supplies. As the COVID-19 crisis continues, we still need to continue to take all necessary measures – COVID appropriate behavior by one and all  – masks, hand hygiene, cough etiquette, physical distancing – and core public health measures by the authorities – detect, test, trace, isolate and treat – to prevent the virus from spreading and causing more disease and deaths.

Community engagement and participation – both for continued COVID-19 appropriate behavior and vaccination – will be critical to curtail the virus transmission.

WHO continues to work with all countries for COVID-19 vaccination planning and roll out, to have a robust National Deployment and Vaccination Plan covering all elements of planning and management needed to deliver a vaccine.

The network of surveillance officers in countries such as Bangladesh, India, Indonesia, Myanmar and Nepal, which was the backbone of polio eradication programme, is now also in the forefront supporting coordination, preparedness and roll out of COVID-19 vaccination down to the district level.

Bringing in best practices from polio eradication, WHO has supported countries in the Region with development of operational guidelines and plans for COVID-19 vaccination; training of vaccinators; planning vaccine and logistics management; and monitoring key preparatory activities.

At the global level, the ACT-Accelerator partnership launched by WHO and partners, has supported the fastest, most coordinated, and successful global effort in history to develop tools to fight COVID-19. The vaccine pillar – COVAX – co-led by WHO, GAVI and the Coalition for Epidemic Preparedness Innovations, aims to accelerate the development and manufacture of COVID-19 vaccines, and to guarantee fair and equitable access.

Orientation to National Deployment and Vaccination Planning for COVID-19 Vaccines

Overview

This course is intended to support national and sub-national focal points in preparing for COVID-19 vaccination. It provides an overview of key aspects of the guidance on developing a national deployment and vaccination plan for COVID-19 vaccines and highlights available and upcoming resources for COVID-19 vaccine introduction.

The course will cover the following areas: regulatory preparedness, planning and coordination, costing and funding, identification of target populations, vaccination delivery strategies, preparation of supply chain, management of health care waste, human resource management and training, vaccine acceptance and uptake (demand), vaccine safety monitoring, management of adverse events following immunization (AEFI), injection safety, and immunization monitoring systems. The course will be updated as additional information becomes available.

Learning objectives

By the end of this course, you will be able to:

  • describe the key considerations for developing and using a national deployment and vaccination plan;
  • describe strategies for the deployment, implementation and monitoring of COVID-19 vaccines in-country;
  • list existing and upcoming resources to support COVID-19 vaccine introduction; and
  • identify key activities to prepare for COVID-19 vaccine introduction.

Course duration: Approximately 4 hours.

Certificates: A Confirmation of Participation is available to participants who complete 100% of the course material.

Course contents

Module 1: Introduction to National Deployment and Vaccination Plan (NDVP) guidance document for COVID-19 vaccines:

This module introduces the National Deployment and Vaccination Plan (NDVP) guidance document for COVID-19 vaccines.

Module 2: Regulatory preparedness:

This module explains key components of regulatory preparedness for timely decision-making by National Regulatory Authorities (NRAs) during public health emergencies.

Module 3: Planning and coordination:

This module explains how countries can use coordination mechanisms to manage deployment and vaccination operations for COVID-19 vaccine introduction.

Module 4: Costing and funding: Ensuring funds reach the point of delivery:

This module discusses how to prepare a realistic budget to enable COVID-19 vaccine deployment and vaccination in consideration of existing essential health services.

Module 5: Identification of target populations:

This module describes the role of NITAG for COVID-19 review, how target populations are defined, and a process for ensuring equity in vaccine access.

Module 6: Vaccination delivery strategies:

This module describes strategies that can be used to deliver COVID-19 vaccines to different target populations.

Module 7: Preparation of supply chain and management of health care waste:

This module describes critical supply chain activities to prepare for vaccine deployment and how to manage health care waste and discusses how to access available tools and resources on supply chain and management of health care waste.

Module 8: Human resource management and training:

This module describes the steps involved in addressing human resource requirements for rolling out the COVID-19 vaccine, including training and supervision.

Module 9: Vaccine acceptance and uptake (demand):

This module describes a data-driven approach to planning, implementing and evaluating demand strategies, describes strategic communication activities that promote COVID-19 vaccination and how to manage expectations, and explains how building trust and managing misinformation is key to achieving acceptance and uptake of COVID-19 vaccines.

Module 10: Vaccine safety monitoring, management of adverse events following immunization (AEFI) and injection safety:

This module explains the unique and different contexts in which pharmacovigilance will have to be implemented for COVID-19 vaccination, as well as the key aspects of injection safety.

Module 11: Immunization monitoring systems:

This module describes vaccination data needs, and explains how to monitor the implementation of COVID-19 vaccination.

COVID-19 vaccination training for health workers

Overview

WHO is working in collaboration with scientists, businesses and global health organizations to speed up the pandemic response and facilitate the equitable access and distribution of COVID-19 vaccines. This course provides general information on COVID-19 and specific information on storage, handling and administration of the vaccine, recording and monitoring including for adverse events following immunization (AEFI), and communication (acceptance and demand) through a series of short video lectures and quizzes to test your knowledge. This course is primarily for frontline health workers who will be vaccinators and priority recipients.

Learning objectives:

By the end of this course, you will be able to:

  • describe the pathogen (SARS-CoV-2) and coronavirus disease (COVID-19);
  • explain cold-chain requirements, storage and handling principles for various delivery contexts, and describe options/procedures for waste disposal;
  • describe the process of COVID-19 vaccine administration and identify infection prevention and control measures that should be used during vaccination sessions;
  • identify an adverse event following immunization and explain how to report AEFI following COVID-19 vaccination;
  • recognize recording and registration forms and explain how to track defaulters; and
  • demonstrate effective and individualised communications about COVID-19 vaccination.

Course duration: Approximately 3 hours.

Certificates: Participants who score 80% or higher on the course quizzes will receive a Record of Achievement certificate.

This course was produced in collaboration with UNICEF. All content has been technically cleared by WHO.

Course contents

Module 1: Introduction to COVID-19 vaccination training:

This is an introduction to the course, the objective of which is to provide key information on COVID-19 vaccination to health workers/vaccinators.

Module 2: Storage, handling, delivery, and waste management of COVID-19 vaccines:

This module consists of three sections, which provide knowledge about: the programmatic implications of COVID-19 vaccines; managing storage, transport, and delivery of COVID-19 vaccines; cold-chain management at the vaccination post and waste management at the facility level.

Module 3: Organizing COVID-19 vaccination sessions:

By the end of this module, you should be able to: outline infection prevention and control measures during vaccination sessions to protect health workers, vaccine recipients, and the community; describe how you prepare for vaccination sessions according to infection prevention and control protocols (at the health facility, for outreach, and for a campaign); and outline the process of safe COVID-19 vaccine administration and waste disposal.

Module 4: AEFI monitoring for COVID-19 vaccination:

By the end of this module, you should be able to: identify an adverse event following immunization (AEFI); describe how to identify and respond to AEFI; and explain how to report AEFI following COVID-19 vaccination.

Module 5: Recording and monitoring COVID-19 vaccination:

By the end of this module, you should be able to: explain the importance of proper recording and reporting; recognize recording and registration forms; correctly record vaccination data and register vaccinations in the reporting forms; and explain how to track defaulters (if applicable).

Module 6: Communication with the community about COVID-19 vaccination:

By the end of this module, you should be able to: describe health workers’ roles and responsibilities for COVID-19 vaccination; demonstrate effective and individualised communications about COVID-19 vaccination; explain communication strategies for three potential scenarios with community members; and explain the role of a vaccinator in crisis communication for adverse events following immunization (AEFI).

Steps for application submission

  • Open URL : http://noc.moest.gov.np
  • Locate and Click on Apply NOC
  • Enter your phone number and email Respectively and click on proceed
  • Enter OTP(One Time Password) from your mobile phone’s message box and click on the Proceed button.
  • Click on the Resend button if you don’t get a message within 2 minutes.
  • Enter your password , confirm it and click on the submit button.
  • Verify your email address and click on the request token.
  • Open Email and enter the email verification token from your email and click on the verify email.
  • Fill all the fields and upload documents and click on proceed.
  • Add your passport details and click on Confirm.
  • Select your previous academic country.
  • Select your previous Academic level, when selected faculty or stream will appear on the screen and select your stream.
  • Select the country Name from the list in the dropdown menu and list of colleges will be seen on the screen.
  • Select the college or university from the list.
  • Select Level of study, Stream/faculty and the subject you want to apply to,
  • select self if you’re applying yourself or select consultancy if you are applying from consultancy and click on proceed.
  • [Note (only if college, university, subject of your choice ):click on Request New. Type your University name, level, faculty/Stream name subject of your choice and click on request.]
  • Click on Proceed to apply for your NOC and success message will appear on your screen.

Nepal Journals OnLine (NepJOL)

Overview

Nepal Journals OnLine (NepJOL) is a database of journals published in Nepal. It covers the full range of academic disciplines. It was initiated in June 2006 and officially launched in September 2007.

NepJOL is managed by the Tribhuvan University Central Library and it is hosted by Ubiquity Press. It is also part of the Ubiquity Press Network. NepJOL provides information on each participating journal, including aims and scope, contact details and general information. It also provides Tables of contents and abstracts (where available) for all articles published within these journals. Many full text articles are also available.

NepJOL uses the Open Journals System created by the Public Knowledge Project based in Canada. This software is open source and allows the journal content listed on NepJOL to be indexed through Open Archives Initiative search engines dedicated to research, which harvest the metadata for each journal article, making this work readily available to a global audience, and giving the journals greater visibility among the worldwide research community.

To give greater visibility to the participating journals, and to the research they convey.

Criteria for Journal selection for inclusion on NepJOL

  1. They are scholarly in content, and contain original research (in addition to other content)
  2. Their content is peer reviewed and quality controlled
  3. They are able to provide all content for inclusion on NepJOL (tables of contents, abstracts and PDFs of full text) in electronic format
  4. They are published within Nepal. Management of publishing strategy, business development and production operation are all run from Nepal.

  • All the material on NepJOL is free to view, search and browse, however copyright of all content is retained by the journals or authors – each journal will need to give permission for any use or re-use of the content that falls outside Fair Use.

Major Achievements

The major achievements of progress in FY 2019/20 are summarised below:

  • The National Health Policy, 2076 (2019) has been endorsed by the Cabinet of Ministers. The policy includes 25 policy statements, each with multiple strategies.
  • The Cabinet of Ministers has endorsed the Public Health Service Regulations 2020, the Safe Motherhood and Reproductive Health Rights Regulations 2020 and the Health Insurance Regulation 2019.
  • Health human resources have been deputed as per the federal structure of governance. An adequate response to the COVID-19 pandemic has been provided.
  • Health services are offered from health facilities despite the challenges posed by COVID-19 Pandemic.
  • Gender-responsive Budget Guidelines for the health sector was developed and approved. Prepared the final draft of National Strategic Plan for 2021–2026 (National Tuberculosis Programme) has been prepared.
  • Pre-bid and post-bid information systems, including Market Analysis (MA), electronic Technical Specification Bank (e-TSB) strengthening, electronic Logistics Management Information System (e-LMIS) reforms, Grievance-handling and Redressal Mechanism (GHRM) installation and online electronic Consolidated Annual Procurement Plan (e-CAPP) modules in the Transaction Accounting and Budget Control System (TABUCS) have been endorsed and implemented.
  • As agreed in the NJAR 2019, the concept of transforming the Procurement Improvement Plan (PIP) into an umbrella strategic document on procurement and Supply Chain Management (SCM) is being progressed by drafting the Nepal Health Sector Public Procurement Strategic Framework (NHSPPSF).
  • Updating of the Financial Management Improvement Plan FMIP into the Public Financial Management Strategic Framework (PFMSF) has been completed and the PFMSF is in the process of being endorsed.
  • Two sets of Standard Operating Procedures (SOPs) for procurement and electronic Government Procurement (e-GP) have been endorsed in FY 2018/19 and their implementation continues across the three levels of government.
  • Emergency Procurement Guidelines have been drafted and are now in the course of endorsement by MoHP.
  • Standardisation of the procurement process through new Standard Bid Documents (SBDs) and e-GP-II implementation in the bidding process has been improved and put into practice. This electronic bidding system, executed as e-GP in FY 2019/20, has increased the proportion of procurement carried out electronically to the highest recorded level: 98 per cent of CAPP value, compared to 83 per cent in e-GP for FY 2017/18.
  • Audit queries against total audit amount have been reduced from 7.01 per cent (in FY 2016/17) to 4.77 per cent (in FY 2017/18). However, this figure increased to 6.73 per cent (in FY 2018/19).
  • Charts of activities have been prepared and are ready to be used in TABUCS.
  • Five district hospitals have been upgraded to 25-bed hospitals, while all remaining district hospitals have been upgraded to 50-bed hospitals. Provincial hospitals are being upgraded to have 200 beds and hospitals under the federal government are being upgraded to have 500 beds.
  • As per the Government of Nepal’s (GoN’s) policy of ‘one municipality-one hospital’, budget has been sent to 396 local levels to establish 5-, 10- and 15-bed basic health care hospitals. Foundation stones for more than 300 hospitals were laid down on a single day (November 30, 2020).
  • Establishment of a 300-bed communicable diseases control hospital at federal level, and similar 50-bed hospitals in each province, has been initiated.
  • The work for establishing 10 trauma centres is ongoing (5 million Nepalese Rupees (NPR) has been allocated to each hospital), and free emergency services are being provided from 14 public sector hospitals.
  • Extensive and unprecedented response actions are being undertaken to mitigate the effects of COVID-19.
  • The Programme Implementation Guideline for FY 2020/21 (for the programme of provincial- and local-level activities) was prepared and made public through the MoHP website.
  • About 3.3 million people in total have enrolled in the health insurance scheme, which is being implemented in 60 districts and 471 local levels.
  • The MA of Pharmaceutical Products in Nepal was designed, and accompanying survey completed, by October 2019. The final report was distributed in FY 2019/20 to all stakeholders.
  • In response to COVID-19, technical specifications of equipment to be installed in the e-TSB have been prepared.
  • The Federal CAPP (FCAPP) was prepared and endorsed in FY 2018/19 and implementation progress is being monitored by the Public Financial Management (PFM) Committee of MoHP and CAPP Monitoring Committee of DoHS. Online preparation of the federal e-CAPP was initiated in FY 2018/19 and the executed e-CAPP for FY 2019/20 captures 97 per cent of the total procurement budget of the MoHP.
  • The Internal Control Guidelines have been drafted as the New Financial Procedures and Fiscal Accountability Act, 2076 (2019) and Financial Comptroller General Office (FCGO) directives.
  • The FMIP has been updated as the Nepal Health Sector Financial Management Strategic Framework, to guide financial management procedures. The framework was endorsed by the Minister of MoHP (on 2077.04.04/19 July 2020) and has been printed and disseminated; its implementation continues across federal-level entities.
  • The Financial Management Reports (FMRs) for each trimester of FY 2019/20 were submitted on time. Improved internal control through internal and final audit clearance was evidenced in the Audit Status Report prepared in August 2020, which was disseminated to External Development Partners (EDPs).
  • Provincial reviews of the health sector in all seven provinces have been completed.
  • MoHP secured funding from the Global Environment Facility to implement the project entitled Building Resilience of Health Systems in Asian Least Developed Countries to Climate Change.
  • Seven additional One-stop Crisis Management Centres (OCMCs) have been established in the first quarter of 2077/78 (2020/21), which makes a total of 77 OCMC sites in 74 districts. Similarly, six Social Service Units (SSUs) were established in the first quarter of 2077/78 in referral and district-level hospitals; the total number of SSUs has gone up to 44.
  • Geriatric health services are available in 24 hospitals as of the first quarter of 2077/78.
  • Health Emergency Operations Centres (HEOCs) are functional in each of the seven provinces, cluster coordination mechanisms activated, and different guidelines/SOPs developed which, played a crucial role in the management of the COVID-19 response.
  • Reconstruction of the structures of 15 Health Posts (HPs) damaged by the earthquake has been completed by National Reconstruction Authority (NRA).
  • An Annual Report on Population for FY 2018/19 has been produced, highlighting major progress in this sub-sector.

The National Centre for AIDS and STD Control (NCASC) has published The HIV Standard Service Package(SSP) For Key Populations, which aims to address services for these populations, providing the standard prevention, diagnosis, treatment and care.

This HIV Standard Service Package (SSP) for KPs was created to document and standardize the effective and innovative approaches to HIV service delivery implemented in Nepal. The standard service package is expected to be implemented by all stakeholders working in HIV service delivery.

Neglected tropical diseases in the context of the COVID-19 pandemic: impact and guidance

Overview

This course is intended for decision-makers and programme managers who want to learn more about neglected tropical diseases (NTDs) in the context of the COVID-19 pandemic. Its 5 modules introduce NTDs, outline the impact of COVID-19 on NTD programmes and WHO’s response to mitigate its consequences, and finally present WHO’s recommendations on maintenance of essential health services for NTDs as well as guidance on adaptation and safe resumption of community-based NTD activities during the pandemic.

Learning objectives

By the end of this course, participants should be able to:

  • describe the main characteristics and disease burden of NTDs;
  • list prevention and control measures for NTDs, within the framework of the 5 strategies recommended by WHO;
  • discuss the impact of COVID-19 on NTD programmes and on transmission of NTDs, as well as WHO’s response;
  • understand how to maintain essential health services in the COVID-19 pandemic context;
  • understand how to adapt facility-based and community-based health care; and
  • understand how to resume mass treatment, active case-finding and population-based surveys for neglected tropical diseases in the context of the COVID-19 pandemic.

Course duration: Approximately 1 hour.

Certificates: A Record of Achievement certificate will be available to participants who score at least 80% in the final assessment.

Course contents

Module 1: Introduction and overview of the course:

By the end of this module, you should be able to: describe how the COVID-19 pandemic has impacted Neglected Tropical Disease (NTD) activities; and explain how NTD activities can be adapted to ensure safe implementation in the context of the COVID-19 pandemic.

Module 2: Impact of the COVID-19 pandemic on NTD programmes and activities:

By the end of this module, you should be able to: describe how the COVID-19 pandemic has impacted Neglected Tropical Diseases (NTDs); and describe WHO’s response to NTDs in the context of COVID-19.

Module 3: WHO general guidance on implementation of NTD interventions in the context of the COVID-19 pandemic and WHO Guidance for community-based interventions:

By the end of this module, you should be able to: describe general guidance for Neglected Tropical Diseases (NTDs) in the context of the COVID-19 pandemic; and describe recommendations for community-based interventions for NTDs in the context of the COVID-19 pandemic.

Module 3: WHO general guidance on implementation of NTD interventions in the context of the COVID-19 pandemic and WHO Guidance for community-based interventions:

By the end of this module, you should be able to: explain the decision-making process for the resumption of NTD community-based interventions; describe the criteria and considerations for the risk-benefit assessment that should be conducted to decide if the planned NTD activity should resume or commence; and describe precautionary measures that can be applied to reduce the risk of COVID-19 transmission associated with the planned NTD activity.

Module 5 : WHO guidance for maintenance of essential health services:

By the end of this module, you should be able to: describe the principles of maintaining essential NTD health services; and explain how NTD programme activities can be modified for the safe delivery of services.
