WHO launches a second report of country success stories in mitigating the impact of the COVID-19 pandemic on TB services

27 April 2022 | Geneva: The World Health Organization (WHO) Global TB Programme has shared its second report of country success stories in mitigating the impact of the COVID-19 pandemic on TB services.   

This new report demonstrates how countries, partners and communities are successfully innovating to adapt to the constraints of the pandemic including through operationalization of dual testing for TB and SARS-CoV-2 (“bidirectional screening”) or using COVID-19 vaccination programmes to screen for TB, sustaining real-time surveillance to improve TB detection and digital interventions to support treatment adherence and reduce health facility visits.

These new country experiences add to the TB programmatic approaches that were summarized in the first WHO report released in 2021. These stories demonstrate that, faced with a crisis, countries in all WHO regions have adopted innovative interventions to maintain and improve TB service provision. 

“The case studies highlight the significant power of innovation in times of crisis, these include the use of digital technologies, community engagement, public–private partnerships and intersectoral approaches to enhance health-care delivery,” said Dr Tereza Kasaeva, Director of WHO’s Global TB Programme “We need to build on these lessons and innovations to improve access to essential life-saving TB prevention and care services, and strengthen pandemic preparedness.”   

WHO urges countries to restore access to and provision of essential TB services such that levels of TB case detection recover to at least pre-pandemic levels. Please consult the WHO TB/COVID-19 Information Note for more details on COVID-19 considerations for TB care.

World Malaria Day: “Harness innovation to reduce the malaria disease burden and save lives”

The theme of this year’s global event highlights the need to harness innovation to reduce the malaria burden and save lives – a theme which fits well with the Region’s “Sustain. Accelerate. Innovate” vision.

For several years now, global progress against malaria has stalled, and in some countries reversed – a trend that has been exacerbated by the COVID-19 pandemic. The latest World Malaria Report highlights an estimated 241 million malaria cases and 627 000 malaria deaths worldwide in 2020. About 14 million more cases and 69 000 more deaths were reported in 2020 compared with 2019. Approximately two-thirds of these additional deaths were linked to disruptions in the provision of malaria prevention, diagnosis and treatment services during the pandemic. In view of recent data and trends, two critical milestones of WHO’s Global Technical Strategy for Malaria have been missed: reducing malaria case incidence and death rates by at least 40% by 2020 compared to 2015 levels. It should therefore come as no surprise that progress towards the Sustainable Development Goal target of ending malaria globally by 2030 is also off track.

The picture in our Region – which accounts for 38% of the malaria burden outside of the WHO African Region – is somewhat brighter. By the end of 2020, the Region had met each of the Global Technical Strategy milestones for mortality and morbidity. We were the only region to do so. In the same year, Bhutan, DPR Korea, Nepal and Timor-Leste reported zero indigenous malaria deaths, while both Maldives and Sri Lanka maintained their malaria-free status. Despite minor setbacks, eliminating malaria continues to be a real possibility in the five countries of the Region actively pursuing it – namely, Bhutan, DPR Korea, Nepal, Thailand and Timor–Leste.

Over the last 20 years, case incidence in the Region has fallen from 18 cases per 1000 population to 3, while total cases have reduced by 78%, from 22.9 million to 5 million. The Regional mortality rate has been slashed from 2.8 deaths per 100 000 population to 0.5, and total deaths have reduced by 75% from 35 000 to 9000.

As a Region, we have made massive achievements – achievements of which we can be immensely proud. We nevertheless have a long way to go to eliminate malaria in all countries of the Region by or before 2030, in line with the Region’s 2017 Ministerial Declaration on Accelerating and Sustaining Malaria Elimination, as well as the 2018 Ministerial Call for Action to eliminate malaria in the Greater Mekong Subregion.   

In high-burden countries, course corrections are needed, for which increased funding will be essential. In low-burden countries, or countries which have eliminated malaria, vigilance will continue to be crucial to prevent a resurgence, especially in border areas – the focus of a key meeting in December 2021 which was supported by the Government of Bhutan.

Together, we must continue to accelerate implementation of the key strategic interventions of the Regional Action Plan 2017–2030, including – First, ensuring universal access to malaria diagnosis and treatment by enhancing and optimizing case management. Second, ensuring universal access to malaria prevention by enhancing and optimizing vector control. Third, transforming malaria surveillance by increasing the sensitivity and specificity of surveillance systems to detect, characterize and monitor all cases and manage foci. And fourth, accelerating efforts to attain and maintain elimination in all countries of the Region.

The way forward is clear. Member States that have eliminated malaria must be better supported to remain malaria-free, with a focus on preventing the importation of new cases. Member States that are within reach of elimination must be supported in achieving and sustaining their goal. High-burden countries with diverse geographical and population contexts must take subnational and locally relevant approaches, ensuring a speedier trajectory that prioritizes the hard-to-reach and underserved, and those at risk of being left behind. Across each of these areas, investment and innovation will continue to be crucial, with a focus on new vector control approaches, as well as better diagnostics and antimalarial medicines. Drug resistance must be addressed sooner rather than later.

WHO remains committed to the SEA Action Plan 2017–2030 towards a Malaria-free South-East Asia Region”, which offers a roadmap to build on our achievements and challenges head-on. I thank countries and partners for their ongoing support, and I reiterate my support to you in turn. I urge all stakeholders to not just sustain but accelerate momentum, leveraging the full impact of existing tools while continuing to look ahead, towards in the innovation pipeline. On this World Malaria Day 2022, we must lay the foundation for a healthier, more equitable and more prosperous Region by eliminating malaria once and for all.

Nepal Health Research Council with support of World Health Organization (WHO) Country Office for Nepal is organizing the Training Workshop on Operational Research on Climate Resilient Health System in Nepal, 03 – 08 May 2022. The aim of this workshop is to enhance country capacity on operational research on climate resilient health system involving both researchers and policy makers in Nepal. Interested participants for the workshop are requested to submit their brief CV and one-page application highlighting motivation for participation and future availability for research and training workshops. The deadline for submission is on 26 April 2022.

The eligibility for participants:

  • At least Master Degree in Environmental Sciences, Epidemiology, Public Health or Statistics
  • At least three years professional experience either teaching, health service or research

World Immunization Week 2022: Long Life for All

Overview

The World Immunization Week observed each year in the last week of April. It aims to promote the use of vaccines to protect people of all ages against disease.

World Immunization Week  aims to:

  • Reframe the global vaccine conversation to focus on the importance of vaccines.
  • Highlight the many ways in which vaccines enable us to live healthy, productive lives by preventing the spread of vaccine-preventable diseases.

Immunization saves millions of lives every year and is widely recognized as one of the world’s most successful and cost-effective health interventions. Yet, there are still nearly 20 million children in the world today who are not getting the vaccines they need.

The main goal of the campaign is to urge greater engagement around immunization globally and the importance of vaccination in improving health and wellbeing of everyone, everywhere throughout life.

Democratic Republic of Congo declares new Ebola outbreak in Mbandaka

Brazzaville/Kinshasa, 23 April 2022 – The health authorities in the Democratic Republic of the Congo today declared an outbreak of Ebola after a case was confirmed in Mbandaka, a city in the north-western Equateur Province. This is the third outbreak in the province since 2018.

So far, just one case has been confirmed. The patient, a 31-year-old man, began experiencing symptoms on 5 April and after more than a week of care at home, sought treatment at a local health facility. On 21 April, the patient was admitted to an Ebola treatment centre for intensive care but died later that day. Having recognized the symptoms, health workers immediately submitted samples to test for Ebola virus disease. Investigations to determine the source of the outbreak are ongoing.

“Time is not on our side,” said Dr Matshidiso Moeti, the World health Organization (WHO) Regional Director for Africa. “The disease has had a two-week head start and we are now playing catch-up. The positive news is that health authorities in the Democratic Republic of the Congo have more experience than anyone else in the world at controlling Ebola outbreaks quickly.”

The Democratic Republic of the Congo is experiencing its fourteenth Ebola outbreaks since 1976. The current outbreak is the sixth since 2018 alone – the most frequent occurrence in the country’s Ebola history. Previous outbreaks in Equateur Province were in 2020 and 2018, with 130 and 54 recorded cases respectively.

Efforts to stem the current outbreak are already underway. The deceased patient received a safe and dignified burial, which involves modifying traditional funeral ceremonies in a way that minimizes the risk of contagious fluids infecting attendees. Anyone who came in contact with the patient are also being identified and their health will be monitored. The health facility where the patient received care has been decontaminated.        

WHO experts based in the Democratic Republic of the Congo are supporting the national authorities to ramp up key outbreak response areas including testing, contact tracing, infection prevention and control, treatment as well as working with communities to support the public health measures to prevent infections.

Vaccination is set to kick off in the coming days. The country already has stockpiles of the rVSV-ZEBOV Ebola vaccine available in the cities of Goma and Kinshasa. Vaccines will be sent to Mbandaka and administered through ‘ring vaccination’ strategy—where contacts and contacts of contacts are vaccinated to curb the spread of the virus and protect lives.

“Many people in Mbandaka are already vaccinated against Ebola, which should help reduce the impact of the disease,” said Dr Moeti. “All those who were vaccinated during the 2020 outbreak will be revaccinated.”

Ebola is a severe, often fatal illness affecting humans and other primates. Case fatality rates have varied from 25% to 90% in past outbreaks. There is now effective treatment available and if patients receive treatment early, as well as supportive care, their chances of survival improve significantly.

22 April 2022 | Kathmandu, Nepal – The Director-General of World Health Organization (WHO), Dr Tedros Adhanom Ghebreyesus, and WHO South-East Asia Regional Director, Dr Poonam Khetrapal Singh, had a courtesy call with the Rt. Hon. President, Bidya Devi Bhandari, and Rt. Hon. Prime Minister, Sher Bahadur Deuba, on day two of their visit to Nepal.  

The WHO leaders were accompanied by WHO Representative to Nepal, Dr Rajesh Sambhajirao Pandav, and Dr Shambu Acharya, Director of Department of Country Strategy and Support.

During the meeting, Rt. Hon. President, Bidya Devi Bhandari, highlighted the importance of multisectoral collaboration in improving health outcomes for the country. 

Dr Tedros commended Nepal’s recognition of health as a basic human right in its constitution.

In their meeting with Rt. Hon. Prime Minister, Sher Bahadur Deuba, Dr Tedros lauded Nepal’s overall COVID-19 response and efforts towards achieving Universal Health Coverage.

Dr Poonam Khetrapal Singh congratulated Nepal for prioritization of health in the national budget and noted how the investment has yielded tangible returns.  

Prime Minister Deuba thanked WHO for its leadership in dealing with the COVID-19 pandemic and urged WHO to further support Nepal towards enhancing the capacity of healthcare workers, strategy development for elimination of cervical cancer, and introducing the HPV vaccine in the country.  

The delegation also visited a school in Lalitpur to observe the ongoing Typhoid Conjugate Vaccination campaign.  The campaign aims to reach 7.5 million children aged 15 months to 15 years with the vaccine and was launched with support from GAVI, the Vaccine Alliance, WHO, and UNICEF on 7 April. With high vaccine acceptance across the nation, the campaign is progressing at a brisk pace. 

The dignitaries then visited the genome sequencing laboratory at the National Public Health Laboratory (NPHL), Department of Health Services (DoHS).  With the in-country capacity development of genome sequencing, Nepal is now able to rapidly identify high threat pathogens of public health concerns. It was through the lab at NPHL that the country detected its first case of the Omicron variant.DG congratulated the leadership at NPHL and DoHS for their accomplishments. 

The WHO delegation had called upon Honorable Health Minister Birodh Khatiwada; Honorable Minister for Foreign Affairs, Dr Narayan Khadka; and Rt. Hon. Chairperson of National Assembly, Ganesh Prasad Timilsina; on 21 April.  

WHO Director-General is here upon the invitation of Honorable Minister for Health and Population, Mr. Birodh Khatiwada. The visit will conclude on 23 April.

Overview

The USAID Health System Strengthening Case Competition, supported by the Accelerator, is an opportunity for USAID staff and partners working on health system strengthening (HSS) activities to showcase the work they are doing and the impact that their activities are having on health systems and health outcomes.

Using real-life examples, the Case Competition will allow us to learn what does and does not work when implementing, institutionalizing, and scaling up health system programs and approaches. These case submissions will help inform USAID and its partners’ ongoing work and will contribute to learning syntheses and dissemination under the USAID HSS Learning Agenda to strengthen the global HSS evidence base.

Fogarty Launching Future Leaders in Global Health Research Training Program

Overview

The INSIGHT consortia is one of many consortia that comprise the Fogarty Launching Future Leaders in Global Health Research Training Program. The purpose of the Fogarty programs is to provide mentored global health research training opportunities in low- and middle-income countries (LMICs) for pre-and post-doctoral candidates from the U.S. and LMICs. The program is sponsored by the Fogarty International Center (FIC) and several collaborating Institutes and Centers at the National Institutes of Health (NIH).

This consortium brings together four top-ranked universities, the University of Alabama at Birmingham (UAB), Baylor College of Medicine (BCM), the University of Maryland Baltimore (UMB), and the University of Pittsburgh (UP) and their respective international research training sites in Africa, Asia, and the Americas. Trainee research projects are supported by experienced and dedicated mentors with expertise in clinical, public health, laboratory, and implementation research.

Fellowship Training Program

To help train and foster the next generation of global health scientists, the Fogarty International Center and its partners at the National Institutes of Health have funded four consortia of universities to administer 12-month clinical research training programs.

The INSIGHT program partnership includes the University of Alabama at Birmingham (UAB), Baylor College of Medicine (BCM), the University of Maryland Baltimore (UMB), and the University of Pittsburgh (UP). 

The program aims to provide the next generation of aspiring global health researchers in the U.S. and in LMIC by giving scientists, early in their careers, a mentored research training experience in global health at established biomedical and behavioral research sites.

In 1998, these institutions were among the first implementers of the Fogarty AIDS International Training and Research Program (AITRP) having provided research training that has strengthened the capacity of academic and research institutions and of research scientists globally.

Our program has four Disease Tracks, four Population Tracks, and three Science Tracks that underpin the global health research and priority needs of our partner LMIC institutions. The specific activities and steering committee of the tracks can be accessed by selecting a research track below.

Application Process

Eligibility

To apply for the program, you must meet the following requirements:

  • Have a mentor at a US institution within our consortium (University of Maryland Baltimore, University of Alabama at Birmingham, University of Pittsburgh, Baylor College of Medicine)
  • Have a mentor at one of our approved international partner sites who is aware of your Fogarty application
  • Ability to dedicate 40 hours per week to research for 12 months at the international fellowship site. Outside clinical duties are allowed during this period
  • Available to begin fellowship in July 2022.
    • LMIC trainees will need to be available to spend 2- to 3-months of their fellowship at one of the US consortium universities to participate in additional training and fellowship activities.

US Citizens/Permanent Residents

Postdoctoral trainee eligibility:

  • have completed a doctoral/terminal degree or equivalent in the last 8 years (PhD, MD, DDS, DVM, etc.).
  • completed a doctoral/terminal degree and are currently in or have recently completed a post-doctoral fellowship (T32 fellowship, medical residency, etc.) or advanced degree (MPH, MSc, etc.) within the last 8 years.
  • will have completed a doctoral/terminal degree or equivalent by July 2022.

Predoctoral trainee eligibility:

  • currently enrolled in a doctoral/terminal degree program and have completed at least one year of studies.

You are NOT ELIGIBLE to apply if you:

  • have not completed a doctoral/terminal degree and are not currently enrolled in one
  • have completed a master’s level degree, but not a doctoral/terminal degree.

LMIC Citizens/Permanent Residents

Postdoctoral trainee eligibility:

  • have completed a doctoral/terminal degree or equivalent in the last 8 years (PhD, MD, MBChB, MMed, etc.)
  • completed a doctoral/terminal degree and are currently in or have recently completed a post-doctoral fellowship or an advanced degree (MPH, MSc, etc.) within the last 8 years.
  • will have completed a doctoral/terminal degree or equivalent by July 2022.

You are NOT ELIGIBLE to apply if you:

  • have not completed a doctoral/terminal degree within the last 8 years.
  • are currently enrolled in a doctoral/terminal degree program but, will not complete your degree by July 2022.
  • have completed a master’s level degree, but not a doctoral/terminal degree.

Deadlines:

  • April 8th, 2022: Application deadline for early decision. Applications received by this time will receive decision by April 30th, 2022. We strongly encourage applying early!
  • May 16th, 2022: Regular application deadline. ALL applications must be received in full by this time to receive consideration.
