Tobacco responsible for 20% of deaths from coronary heart disease

Every year, 1.9 million people die from tobacco-induced heart disease, according to a new brief released today by the World Health Organization, World Heart Federation and the University of Newcastle Australia ahead of World Heart Day, marked on 29 September.

This equates to one in five of all deaths from heart disease, warn the report’s authors, who urge all tobacco users to quit and avoid a heart attack, stressing that smokers  are more likely to experience an acute cardiovascular event at a younger age than non-smokers.

Just a few cigarettes a day, occasional smoking, or exposure to second-hand smoke increase the risk of heart disease. But if tobacco users take immediate action and quit, then their risk of heart disease will decrease by 50% after one year of not smoking.

“Given the current level of evidence on tobacco and cardiovascular health and the health benefits of quitting smoking, failing to offer cessation services to patients with heart disease could be considered clinical malpractice or negligence. Cardiology societies should train their members in smoking cessation, as well as to promote and even drive tobacco control advocacy efforts,” said Dr Eduardo Bianco, Chair of the World Heart Federation Tobacco Expert Group.

The brief also shows that smokeless tobacco is responsible for around 200 000 deaths from coronary heart disease per year. E-cigarettes also raise blood pressure increasing the risk of cardiovascular disease.

Moreover, high blood pressure and heart disease increase the risk of severe COVID-19. A recent WHO survey found that among people dying of COVID-19 in Italy, 67% had high blood pressure and in Spain, 43% of people who developed COVID-19 were living with heart disease.

“Governments have a responsibility to protect the health of their people and help reverse the tobacco epidemic. Making our communities smoke-free reduces the number of tobacco-related hospital admissions, which is more important than ever in the context of the current pandemic,” said Dr Vinayak Prasad, Unit Lead of the WHO No Tobacco Unit.

Tobacco control is a key element for reducing heart disease. Governments can help tobacco users quit by increasing tax on tobacco products, enforcing bans on tobacco advertising and offering services to help people give up tobacco.

The Peter Salama Refugee MPH Scholarship  in Humanitarian Health

The Center for Humanitarian Health announces the launch of a refugee scholarship program in honor of the late Dr. Peter Salama, who was a member of the Center’s Advisory Committee. Dr Salama was the Executive Director of WHO’s Division for Universal Health Coverage – Life Course. Before joining WHO in July 2016, Dr Salama was UNICEF’s Regional Director for the Middle East and North Africa.

The scholarship will support one individual who has been displaced due to the conflicts in Afghanistan, Myanmar, South Sudan, Syria, and Venezuela. These students will receive full tuition while pursuing a Master of Public Health degree at the Johns Hopkins Bloomberg School of Public Health.

Eligibility

To be eligible for the Humanitarian Health Scholarship, applicants must also:

  • Be from a low- or middle-income country (LMIC), as defined by the World Bank: https://data.worldbank.org/income-level/low-and-middle-income;
  • Be able to fluently speak, read, and understand English in order to actively participate in a fast-paced English-only academic program;
  • Have completed at least a bachelor’s degree, with a strong record of successful academic performance; and
  • Be professionally involved in humanitarian health emergencies/disasters* for over two years, and plan to continue to work in these contexts after completion of the degree.

NHRC CPD Guidelines for Providing Credit Hours to Participants of Research Trainings and Conferences

NHRC Guideline for CME/CPD Accreditation (For Health Science Professionals)

This guideline is applicable for providing credit hours for health professionals
attending research trainings and participating in health research related
conferences,, summits, workshops, and seminars.

Introduction

The Nepal Health Research Council is a national apical body responsible for
providing scientific study and quality health research in the country with the
highest level of ethical standards. It started as a Nepal Health Research Committee under the Ministry of Health, chaired by the Secretary of Health in 1,982 AD (BS 2039). On 11 April 1991 (29 Chaitra 2047 BS), the committee was developed as the Nepal Health Research Council, a statutory and autonomous body as promulgated by the Nepal Health Research Council Act No. 129 of the year 1991 of Government of Nepal. With the consent of the Council of Ministers under Article 129 of the constitution of the Government of Nepal, 1990 enacted the NHRC. The primary responsibility of the NHRC is to acts as a national regulatory body for maintaining technical and ethical standards of health research within the country.

NHRC also acts to promote and coordinate health research and research communication.

It has been regularly publishing a bio-medical journal and organizing scientific
sessions/ trainings/workshops/summits to make the health professionals fully upto-date with the advances in health science and research.
Continuing Professional Development (CPD) in health science is very essential for quality health research and quality service delivery. With ever increasing
importance of CPD, NHRC in consultation and cooperation with stakeholders like
(Nepal Medical Association (NMA), professional societies, academia and health
ministry: has come up with the establishment of a system of accreditation of
Continuing Professional Development [CPD) credits.

CPD and Aims of CPD

CPD is any learning outside of undergraduate or postgraduate education that helps to maintain and improve performance of health professionals in context of health research. It covers the development of your knowledge, skills, attitudes and behaviors across all areas of health research. It includes both formal and informal learning activities.

CPD activities should maintain and improve:

  • The quality of health research conducted by professionals in different fields.
  • Keep professionals engaged in health research updated about the latest Development in field of health research.
Credit hours to be awarded

Participant will be awarded:

  • One credit points for one hour of participation in trainings.
  • Presenter/Facilitators will be awarded two credit points for one hour of presentation.

National Mental Health Survey, Nepal-2020 Fact Sheet

The National Mental Health Survey, Nepal was started from November 2017 and carried out from January 2019 to January 2020 in all seven provinces of Nepal. The total sample size of the survey was 15088 among which 9200 were adults (aged 18 years and above) and 5888 were adolescents (aged 13-17 years).

Highlights

Mental Disorders Factsheet (Adolescents)

  • The prevalence of mental disorder among adolescents was found to be 5.2%.
  • Neurotic and stress related disorders were the most prevalent, with a prevalence of 2.8%.
  • The prevalence of mental disorders was highest among adolescents in Province 1 (11.4%) among 16-years-olds (7.7%) and among females (5.3%).
  • The prevalence of current suicidal thoughts among adolescents was 3.9% while that of lifetime suicidal attempt was 0.7%.

Mental Disorders Factsheet (Adults)

  • Among the adult participants, 10% had any mental disorder in their lifetime, and 4.3% currently has any mental disorder.
  • Lifetime and current mood disorders among adult participants were found to be 3% and 1.4% respectively.
  • Neurotic and Stress related disorders (current) among adult participants was 3%.
  • The prevalence of suicidality (including current suicidal thoughts, lifetime suicidal attempt and future likelihood of suicidal thoughts) was found to be 7.2%.
  •  Majority of the adult participants (6.3%) has low suicidality compared to moderate and high suicidality.
  • Current suicidal thoughts and Lifetime suicidal attempt were found to be 6.5% and 1.1% respectively.
  • The prevalence of lifetime mental disorders was highest among adults in Province 1 (13.9%), among 40-49-years-old (13.3%) and among males (12.4%).
  • The prevalence of current mental disorders was highest among adults in Bagmati province (5.9%), among 40-49-years old (6.3%) and among females (5.1%).

World Alzheimer’s Day 2020: “Let’s talk about dementia”

Today (September 21st) is World Alzheimer’s Day. The objective of World Alzheimer’s Day is to raise awareness, raise issues faced by people affected by dementia and to support them. World Alzheimer’s Day was launched in 1994.

Theme

The theme of World Alzheimer’s Day 2020 is “Let’s talk about dementia” This year the priority should be given to break down the fear and the stigma and we encourage people to seek out information, support and advice – something which has become acutely necessary during the global COVID-19 pandemic.

Dementia

Dementia is a collective name for progressive degenerative brain syndromes which affect memory, thinking, behaviour and emotion. Alzheimer’s disease and vascular dementia are the most common types of dementia, responsible for up to 90% of cases of dementia.
Symptoms may include:
• loss of memory
• difficulty in finding the right words or understanding what people are saying
• difficulty in performing previously routine tasks
• personality and mood changes

COVID-19 and dementia

  • The COVID-19 outbreak has highlighted the lack of preparedness of health systems
    globally to provide routine services and support to people living with chronic conditions
    such as dementia.
  • During lockdown and restrictions, it’s essential that older people, especially those living
    with dementia, are not socially excluded.
  • During the COVID-19 pandemic, it is imperative that people keep talking about dementia,
    seeking out information, advice and support.
  • Lack of knowledge about dementia leads to inaccurate assumptions about its effects on
    the person and their family and negative stereotypes about how a person with dementia
    will behave.
  • Talking about dementia leads to better planning and to the access of supports and services to improve the life of people living with dementia and their carers and families.
  • Stigma and negative attitudes around dementia persist in every country. This World Alzheimer’s Month, we want to talk about it.
Statistics
  • There are over 50 million people around the world living with dementia.
  • Someone in the world develops dementia every 3 seconds.
  • The number of people living with dementia is predicted to triple, rising to 152 million by
    2050.
  • The economic burden of dementia is US$1trillion dollars every year, a figure that will
    double by 2050.
  • Almost 80% of the general public are concerned about developing dementia at some
    point and 1 in 4 people think that there is nothing we can do to prevent dementia.
  • 35% of carers across the world said that they have hidden the diagnosis of dementia of a
    family member.
  • Over 50% of carers globally say their health has suffered as a result of their caring responsibilities even whilst expressing positive sentiments about their role.
  • Almost 62% of healthcare providers worldwide think that dementia is part of normal ageing.
World Alzheimer’s Month

September is World Alzheimer’s Month, an international campaign to raise dementia awareness and challenge stigma. Each year, Alzheimer associations from around the world unite to organise advocacy and information provision events, as well as Memory Walks and fundraising days.

Psychology: The Hidden Side of Pandemics

Pandemics are viral in nature, their onset initiates from animals and spreads to humans. Homo sapiens have had to endure numerous pandemics throughout history such as Plague of Justinian (541-750 AD), Black Death (1347-1351), Spanish Flu (1918-1919), Hong Kong Flu (1968-1970), SARS (2002-2003), Swine Flu (2009-2010), Ebola (2014-2016), and currently coronavirus (COVID-19).

These are some of the most severe and deadly pandemics in the entire human civilization to have victimized humans with pain and despair. Pandemics are larger scale epidemics affecting millions of people across multiple countries with the capability of spreading across globe – WHO.  The virus is supposedly brought to humans by animals, it is known that 20% of such infected humans despite remaining contagious do not appear to show symptoms of any sort, thereby leaving the community greatly exposed to risks of contraction. It is believed that those 20% in turn infect further 80%, signifying the extraordinary pace in which the virus is capable of spreading.

Pandemic show a number of visual sides. In addition to physical symptoms and hospitalization and eventually the prospects of being cured, facts like deaths of uncured, economic and political strain to individual and communities are worth a note. Hence, it is important that psychological reactions to the infection be considered as integral to spreading and contamination of the disease.

Pandemics by its nature exert significant psychological pressures to individuals, these pressures are stresses caused directly by the pandemic, related to uncertainty of course of pandemic. Health threats to self and loved ones appear to be one of the prominent reasons to these stresses, likewise, failing to maintain a routine life and rituals due to limitations placed on movements compounds the stresses.  The social isolation situation amid lockdown and quarantine creates fear of wage and job loss too, and in events, lengthy economic inactivity and closures can lead to shortage of survival needs and basic medicines, financial hardships might in some cases lead to malnutrition.

Psychoneuroimmunology researches suggest that heightened stress due to pandemic suppresses immune system, consequently resulting in health hazards and disease vulnerability.  The uncertainty around losing near and dear ones leads to trauma, and individuals caring the infected experience extreme stress. Communities find it brutal to think of not being able to attend death ceremonies and perform traditional funeral practices. Moreover, the indirect exposure to distress could to a great length be attributed to news and graphical illustrations of fatalities broadcasted on media.

Studies on the most recent epidemics (SARS 2003 and Ebola 2015) have shown that the epidemic of fear is worse than the epidemic itself, and that the psychological effect lasts longer than somatic effect. These previous experiences have shown how people avoided risk and disregarded health advices by refusing to vaccinate and adhere to hygiene practices and social distancing (currently social distancing is terminologically replaced by physical distancing).  Excessive fear and disability paves way to practice of unhealthy coping mechanisms like reassurance seeking and repetitive checking in response to the threats of infections.  In areas where the epidemic had hit the most people resorted to animal cruelty, as a direct response to assumptions that the perceived source of infections could expand to animals.  Being anxious and high level of stress commonly shows obsessive activities for instance cleansing money and washing hands frequently. Such stress is known to cause mental disorders like PTSD, anxiety disorders, mood disorders and in some cases depression and severe grief. The aforementioned studies, also found how folk remedies and superstitions became prevalent in the absence of scientific solution to the outbreak and its expanding intensity. During the Spanish Flu a Canadian man had been understood to have drunk hydrogen peroxide, reminiscent to that, rumors like the intake of alcohol and garlic are helpful for avoiding covid-19 infection are spreading during coronavirus outbreak.

Negative coping mechanisms get developed during pandemics, the mechanisms of negative emotionality, trait anxiety, overestimation of threat, unrealistic optimism bias are some examples. There are potential tragedies associated with pandemics like doctor shopping, panic buying, social and civil unrest likewise it is cautionary that at time of worldly unrest and crisis conspiracy theories, rumors, fear contagion, mass media and social media have known to create hype on people that makes them panic and heighten public anxiety. The elements surrounding this unprecedented situation are adequate to create a state of stigma.

The effects, risks and repercussions are real and with adequate reflection from similar outbreaks of the past and established body of knowledge, it can be suggested that healthy coping mechanisms be communicated. Instructions for managing stress and building resilience along with ways of dealing with stigma and prejudice can prove to be useful. Another helpful activity can be providing psycho-educational materials on grief, loss, fear, helplessness, hopelessness, panic, neuroticism, anxiety, depression, apathy, anhedonia, frustration, pain, anger, carelessness and more. Knowing the facts regarding the pharmacological treatment and vaccine development might decrease the chaos of pandemics. Similarly, hygiene practice like wearing mask, regular handwashing, use of antiseptics like sanitizers, boosting the immune system i.e. diet therapy and maintaining physical distancing are the key tools for self-care. Engaging in the in-house activities, drawing, gardening, playing music, doing yoga, learning new skills and making chit-chat with family members will certainly ventilate the stress of pandemics.  In case of severity, medicines like Selective Serotonin Reuptake Inhibitor (SSRI) and Cognitive Behavior Therapy (CBT) through trained medical practitioner and psychotherapist is helpful for managing emotional distress.

Do not forget psychologist are not the frontiers of pandemics and should not be trusted too much in pandemics.

International Snakebite Awareness Day 2020

September 19th is International Snakebite Awareness Day (ISBAD). International Snakebite Awareness Day was initiated from 2018. ISBAD provides an opportunity to raise the awareness of one of the world’s biggest hidden neglected health crises ‘Snakebite’.

The International Snakebite Awareness Day call for increasing awareness of the right ways to prevent snake bites, minimize risks and and improve the first aid and primary care of snakebite victims.

Snakebite envenoming is a potentially life-threatening disease caused by toxins in the bite of a venomous snake. In 2017 the WHO added snakebite to its list of Neglected Tropical Diseases (NTDs). 

Key facts

  • The exact number of snake bites is unknown.
    Estimated 5.4 million people are bitten each year.
  • Around 81 000 to 138 000 people die each year because of snake bites.
  • Agricultural workers and children are the most affected.

Nepal

  • Twenty-one of the 79 species of snakes found in Nepal are poisonous (11 pit viper species, 5 krait species, 3 cobra species and 1 each coral and Russel’s viper species).
  • Around 15,000 snake bite cases estimated annually of which about 10 percent are poisonous bites.
  • The mortality rate is about 10 percent among poisonous bite cases. The 26 Tarai districts are highly affected. In the last eight years between 1 and 131 deaths have been reported from poisonous snake bites each year.
  • The free distribution of anti-snake venom serum (ASVS) began in 1999/2000.
  • There are 85 snake bite treatment centres are in the country for snakebite management in collaboration with Nepal army, Nepal Red Cross Society, community members.
  • In 2074/75, altogether 5,606 snake bite cases were reported at national level. A total of 794 cases were poisonous resulting in 20 deaths.
