WHO issues rapid communication on updated guidance for the management of TB in children and adolescents

by Public Health Update August 26, 2021
written by Public Health Update

26 August 2021 | Geneva – A rapid communication released by the World Health Organization (WHO) Global Tuberculosis Programme has announced important updates to guidance on the management of tuberculosis (TB) in children and adolescents. This includes new recommendations on diagnostic options, treatment regimens, as well as treatment decision algorithms and optimal models of care for the delivery of child and adolescent TB services.

“Tuberculosis in children and adolescents has been overlooked for many years,  reflected in large gaps in access to TB prevention and care. Therefore it is encouraging to see that options for the diagnosis, treatment, prevention and care for children and adolescents with TB or at risk of TB, are expanding, thanks to the generation of new evidence” said Dr Tereza Kasaeva, Director of WHO’s Global Tuberculosis Programme. “We ask for ongoing commitment from country officials and stakeholders to prepare for and support the full implementation of new WHO recommendations on the management of TB in children and adolescents, which will soon be outlined in updated guidelines.”

The rapid communication highlights key updates including:

  • A recommendation on the use of Xpert MTB/RIF Ultra in gastric aspirate or stool specimens as the initial diagnostic test for TB and the detection of rifampicin resistance, in children aged below 10 years with signs and symptoms of pulmonary TB.
  • A recommendation to use a 4-month treatment regimen (2HRZ(E)/2HR) rather than the standard 6-month regimen (2HRZ(E)/4HR) in children and adolescents under 16 years of age with non-severe, presumed drug-susceptible TB.
  • In children with MDR/RR-TB of all ages:
    • A recommendation to use bedaquiline as part of the shorter, all oral bedaquiline-containing regimen (conditionally recommended by WHO in 2020) or as part of longer treatment regimens.
    • A recommendation to use delamanid as part of longer treatment regimens.
    • These recommendations make it possible to design all-oral regimens for children of all ages.
  • A recommendation on the use of a shorter intensive regimen composed of 6 months of isoniazid, rifampicin, pyrazinamide and ethionamide in children and adolescents with microbiologically confirmed or clinically diagnosed TB meningitis, presumed to be drug susceptible, as an alternative to the currently recommended 12 month regimen.
  • Treatment decision algorithms incorporating WHO recommended diagnostic tests may be used for children aged below 10 years with signs and symptoms of pulmonary TB. In addition, in high TB burden settings, decentralized and family-centred, integrated services may be implemented to improve TB case detection and the uptake of TB preventive treatment.

The rapid communication aims to inform staff from ministries of health and care providers across public and private sectors, technical partners and other stakeholders about the key findings, considerations and changes related to the diagnosis, treatment and care of TB for children and adolescents, in order to allow for planning at the country level ahead of the release of updated guidelines and an associated operational handbook.

Detailed recommendations will be published in the coming months as part of WHO’s Consolidated Guidelines on Tuberculosis, Module 5: Comorbidities, Vulnerable Populations and People-centred Care, alongside a complementary operational handbook, which will contain implementation guidance. The recommendations are based on the outcomes of a Guideline Development Group meeting.

All applications must be submitted online. In order to help you prepare, you can see our application guidelines here for your reference.

The official language for the application is English.

If you are submitting any supporting documents (such as residency certificate, degree certificate, etc.) in a language which is not English, you must attach an (informal) translation into English. Documents in other languages with no accompanying English translation will not be considered.

Eligibility

The applicant must be a woman who has received her PhD in a scientific discipline within the previous ten years and whose current scientific research is related to the area of climate action and the environment.

Given that from 2022-2026 the awards will respond to the call for action set forth by the United Nations Sustainable Development Goals (SDGs), applicants must demonstrate how their research contributes to the advance of knowledge in the area of climate action and the environment and to achieving the SDG13 (Climate Action), SDG14 (Life Below Water) and/or SDG15 (Life on Land).

The application can include, but is not limited to, reference to SDG13, SDG14 or SDG15 sub-targets, e.g.: effective climate change-related planning, protecting marine and coastal ecosystems, halt biodiversity loss, conserve and sustainably use ecosystems, strengthen resilience to climate-related hazards, and more.

If in doubt about your eligibility, please contact owsd@owsd.net.

Prize

  • Cash prize of USD 5,000.
  • All-expenses-paid trip to attend the Awards Ceremony to be held at the annual meeting of the American Association for the Advancement of Science (AAAS) in February 2022. The trip will be confirmed in the coming months according to how the COVID-19 pandemic evolves; it may be that the awards ceremony is celebrated online. Awardees will not be required to travel if they judge the situation to be unsafe.

The 5 awards will be distributed as follows: one for each of the four regions of the developing world (Africa, Arab region, Asia & the Pacific and Latin America & the Caribbean), plus an additional ¨floating” award for an outstanding candidate from any of these regions.

Applications

Applications for the 2022 awards are invited from women scientists who have made significant contributions in the area of ‘Climate action and the environment’, whose research is linked to the following Sustainable Development Goals: SDG13 (Climate Action), SDG14 (Life Below Water) and/or SDG15 (Life on Land), and have lived and worked for at least 5 years in an eligible STLC.

Applications must be made online, in English, and must include:

  • Official documentation proving 5 years’ residency in an eligible STLC .

The following documents can be submitted as proof of residence as long as the name of the applicant (as written in the online form) appears clearly: statement from the local police office or municipality, population registry office, rental agreement, utilities bills, employment contract. This list is not exhaustive and alternative documents can be considered. Please write to owsd@owsd.net if you are not sure about the eligibility of your documents.

  • Evidence on how your research contributes to advancing the SDG13 (Climate Action), SDG14 (Life Below Water) and/or SDG15 (Life on Land).
  • Short biography of approximately 350 words describing your journey to become an outstanding scientist (for ease, we recommend that you prepare this beforehand and copy-paste into the text box)
  • PhD certificate
  • Curriculum vitae
  • Full list of publications (if not included in the CV)
  • At least 2 reference letters (reference letters must be prepared on official letterhead and should be written and signed by senior scientists familiar with your research).

The deadline for submission of applications for the 2022 awards is 28 October 2021.

Application guidelines and a sample application form can be found here.

Selection

The competition will be judged by a distinguished panel of specialists in the appropriate fields and who are aware of the challenges facing women scientists in developing countries.

Traditionally, the selection committee has met in person at the OWSD Secretariat based in Trieste, Italy and we have been able to draw on a broad range of expertise from this ‘City of Science’, including the International Centre for Theoretical Physics (ICTP), the University of Trieste, Elettra Sincrotrone, The International School for Advanced Sciences (SISSA) and the International Center for Genetic Engineering and Biotechnology (ICGEB). Since 2020, due to the COVID 19 pandemic and the impossibility of holding face-to-face meetings, we had the opportunity to draw on a broader pool of experts based also in the developing world. The Selection Panel is chaired by the OWSD Coordinator with full administrative support from the OWSD Secretariat.

Assessment will be based on the applicant’s achievements in her scientific field, with additional attention paid to evidence of leadership skills, initiative and innovation, as well as to the candidate’s involvement in capacity building, outreach and civic contribution. Researches that demonstrate the use of innovative techniques or methodologies (especially with regard to sustainable development), an awareness of the importance of sex and/or gender considerations in research methodology and content and/or interdisciplinary collaboration may be prioritised.

Successful candidates will be informed of their selection by 31 December 2021.
