Once-a-week pill for HIV shows promise in animals

Human trials of a once-a-week oral pill for HIV could start, after successful tests in pigs, claim US scientists.
The slow-release tablet could free patients from having to take daily medication, they say.
It looks like a normal capsule, but on reaching the stomach its coating dissolves and a special structure packed inside unfolds.
This 4cm (1.5in) star-shaped scaffold stays in the stomach for seven days, steadily releasing its cargo of drugs.
More tests in other mammals, including monkeys, are recommended but the researchers say trials in people could begin within two years.
HIV experts said the prospect of a new treatment option was to be welcomed, but that a once-a-week HIV pill for people was “still a way off”.
The star is too large to move out of the stomach but still allows food to journey through to the small intestine.
Once it has delivered its payload, the star begins to degrade and passes on through the digestive tract.
In the pig trial, the researchers dosed it with enough of three antiretroviral drugs – dolutegravir, rilpivirine and cabotegravir – to last for seven days.
The researchers say, in the future, the oral drug delivery device could be used for a wide range of diseases, not just HIV.

Slow release

Preliminary tests in pigs have already been done with a malaria drug called ivermectin and the star remaining in the stomach for up to two weeks.
Researcher Giovanni Traverso, from Massachusetts Institute of Technology and Brigham and Women’s Hospital in the US, said: “We wanted to come up with a system to make it easier for patients to stick to taking their treatments.
“Changing a medication so it only needs to be taken once a week rather than once a day should be more convenient and improve compliance.
“Once-a-month formulations might even be possible for some diseases.”
A company called Lyndra is now developing the technology and plans human trials of the long-lasting oral delivery pills within the next 12 months. Tests with HIV medication could begin after that, subject to approval and more animal tests.
Dr Traverso said: “There are lots of patients this could help, including people with dementia or mental health disorders such as schizophrenia.”
Some slow-release drugs could already be given by injection, he said.
A spokeswoman from the British HIV Association said: “This research is still in the early stages of development and there is clearly some way to go from testing in pigs and mathematical modelling to human trials before its effectiveness can be assessed.”
A Terrence Higgins Trust representative said: “Medical advances have come on leaps and bounds for HIV in the UK in recent years, however we do know that taking a pill each day does present practical barriers for some people living with HIV.
“We welcome the prospect of a treatment that removes these barriers, and presents all people living with HIV with further choice, provided that it is no less effective than current options available.”
The HIV research, published in the journal Nature Communications, was funded by the Bill and Melinda Gates Foundation, the National Institutes of Health and the Brigham and Women’s Hospital.

Survey: Data consolidation on Nepalese NCDI studies – The Nepal NCDI Poverty Commission

Nepal NCDI Poverty Commission is a country level platform of NCDI researchers formed in the wake of Global NCDI Poverty Commission (http://www.ncdipoverty.org/). The main objective of the national commission in Nepal is to assess the burden, risk factors and mortality due to key NCDIs (all major NCDs, mental health, respiratory diseases and injuries), and its distribution by socio-economic status (SES) and poverty index. This information will help the national commission develop a package of key interventions addressing NCDIs. The working group one in the national commission is consolidating information on Nepalese NCDI studies. If you are part of any NCDI studies in Nepal which assesses NCDI burden and risk factors to any detail, please let us know by filling out the form below. Your participation is voluntary. The collected information will be valuable to improve data-infrastructure for NCDi studies in Nepal.

Viral fever in Jajarkot still untamed, death toll strikes 13

JAJARKOT: 65-year-old man Kalu Tiwari has scummed to viral fever and common cold in Junichande rural municipality of Jajarkot, on Wednesday. With this total number of the victims to die from the viral fever and common cold has reached to 13 according to District Public Health Office’s Chief Ram Bahadur Nepali. Tiwari had taken his last breath on the way to the hospital.
Meanwhile, the local residents have expressed their dissatisfaction over government’s lackluster efforts to curve the impact of viral fever and common cold. “Every year many people lose their lives due to such calamity, but government always shows its concerns very lately. This time as well, government is not mobilizing enough manpower in the affected areas,” said local journalist Rajendra Karki.
The meeting of District Natural Disaster Rescue Committee led by Chief District Officer held on Wednesday decided to dispatch the medical teams with the needed medicines.
District Public Health Office’s Chief Nepali informed that the samples collected from the affected patients have been sent to Kathmandu. “We have sent the samples to the Epidemiology and Diseases Control Division and the report after assessing the samples will clarify about the diseases,” said Nepali.
He further informed that a total of 1050 patients have already taken the health care service and more than 1000 are yet to get that sort of service.
Meanwhile, all the schools of the Junichande rural municipality have been shut down to prevent students from the viral.
District Education Office informed that a total of 59 schools of the Junichande are closed for the indefinite term.

Infant dies from viral fever in Jajarkot

The deceased has been identified as two-month-old Bhupendra Nepali, son of Ram Bahadur Nepali. A local Mukti Sharma said that the infant died in course of treatment in the house.
According to the family, although a health camp was organised at Bhairab Secondary School, they could not reach there as the school is very far from their house.
Junichande rural municipality in the district is in the grip of viral fever for over a week now which has claimed nine lives so far in the village.

Police arrests Dr Govinda KC from Maharajgunj-based Teaching Hospital

Jan 8, 2018- A team of Kathmandu Police Range on Monday arrested Dr Govinda KC from Maharajgunj-based Tribhuvan University Teaching Hospital, Kathmandu. 
The team left shortly after the Supreme Court issued an order to Nepal Police directing to arrest the agitating doctor and present him before the court on Tuesday morning.
Dr KC will be detained at Singha Durbar Police Circle after the arrest.
Dr KC began his 14th hunger strike at the TUTH premises on Monday demanding resignation of Chief Justice Gopal Parajuli.

Supreme Court orders Dr KC arrested on criminal contempt charge

Kathmandu, January 8
Nepal’s Supreme Court has ordered police to arrest activist Dr Govinda KC on charge of criminal contempt of court.
The order comes a few hours after Dr KC, enraged by the Supreme Court’s order to reinstate Dr Shashi Sharma as the Dean of Tribhuvan University Institute of Medicine, accused Chief Justice Gopal Parajuli of being corrupt.
A bench of Justice Damber Bahadur Sahi issued the order on Monday evening demanding that KC be brought before the court at 9 am on Tuesday.
Dr KC, who has been advocating for reforms in medical education, announced his 14th hunger strike earlier on Monday.

15 per cent infants in Siraha malnourished

LAHAN: Around 15 percent children in Siraha District suffer from dire state of malnutrition, a study revealed.
According to Chuman Lal Das, Chief of District Public Health Office, lack of nutritious diet to pregnant women, heavy workload, lack of rest and negligence from family members have resulted in under-nutrition in 15 percent of the youngsters.
“Around 10 percent of children in Nepal and 14.4 percent of children in Province 2 are subjected to undernourishment,” Das informed, adding “Children from rural and economically backward societies suffer from malnutrition because of lack of nutritious diet whereas children in urban areas are malnourished due to lack of awareness regarding balanced diet.”
The study conducted by “Quick Assistant” on babies up to the age 5 in the district found out that physical, mental and social development of the infants have been affected and they have not gained height and weight in line with their age.
Keeping in mind the impending issue of malnutrition, various donors and health agencies including UNICEF, World Food Programme (WFP), UK Aid, Global Health Alliance Nepal, Nepal Public Health Foundation and Australian Aid in coordination with District Health Office have started an Immediate Malnutrition Unified Management Programee in the District, added Das.

Some drugs supplied by govt, pvt sector are substandard: NHRC

Life-saving drugs supplied to the health facilities and to the market by both the government and the private sector have been found substandard and a danger to public health.
Nepal Health Research Council (NHRC) sent 214 samples of the drugs for in-vitro analysis at two different laboratories in order to examine them for quality. Out of the 214 samples, 26 were supplied by the government and 188  by the private sector. Among these, nine drugs were found to be substandard.
 According to research published recently by  NHRC, three essential drugs supplied by the Government of Nepal were found to be substandard, including Paracetamol 500mg BP (Cetophen). 
“Six non-essential drugs supplied by the private sector also failed to meet the required standards in one laboratory test,” says the NHRC research report.
According to the Department of Drug Administration (DDA), drugs worth Rs 16.5 billion (46 percent) constitute the annual domestic production while drugs worth Rs 19.45 billion (54 percent) annually are imported. Of the 54 percent imported medicines, 52 percent is purchased from India and two percent from other countries.
The government purchases 72 kinds of essential drugs for distribution to the public free of cost through government health facilities. According to the Primary Health Care Revitalization Division (PHCRD) under the Department of Public Health Services, the government has allocated a total of Rs 1.5 billion in the current fiscal year for the purchase of essential medicines that are to be distributed free of cost. 
Out of this total amount, Rs 362.6 million (24.27 percent) is allocated for local bodies, Rs 19.5 million (1.03 percent) for upgraded hospitals, Rs 57.87 million (3.86 percent) for district health/public health offices, Rs 10 million (0.67 percent) for the five regional hospitals and Rs 1.05 billion (nearly 70 percent) for the center.
Similarly, the government allocated Rs 1.49 billion for the procurement of essential medicines last fiscal year and Rs 1.5 billion the previous fiscal year. Out of the annual allocations, 70 percent is allocated for the center and 30 percent for the local levels.
Nine percent of the drugs available in the market are estimated to be substandard, said Santosh KC, senior drug administrator at the Department of Drug Administration (DDA). “A larger number of drugs produced by domestic manufacturers are also substandard,” he added.
KC further said the low quality of raw materials and inadequate level of ingredients compromise the quality of the drugs, and there are also problems of storage and transportation. 
“We are the monitoring authority but checking the market has not been so effective,” KC said, “There is also lack of a comprehensive law on drug procurement. We still purchase medicines under laws meant for construction materials,” he added. “The technical specialty of medicines should be evaluated differently while budgeting for medicines,  but the present practice is to reward tenders to the lowest bidder.”
Bhogendra Raj Dotel, director at  PHCRD, admitted the lack of specific procurement laws in the country. “However, the substandard medicines mentioned by  NHRC were not purchased by the central government,” he claimed. “Medicines purchased at local, district and regional levels might be substandard.”
The NHRC carried out a cross-sectional study of 90 health facilities in the five development regions, including one district hospital, one primary health care center and four health posts from one district each representing the mountain, hill and tarai regions. It also collected 10 generic medicines each of five brands. 
Among the total prescribed drugs from all the health facilities, 60.71 percent were found to have been prescribed with generic names instead of brand names and the highest percentage were from district hospitals, according to  NHRC. A total of 68.91 percent was found to be essential medicines while 23.74 percent was antibiotics. There was an almost 400 percent variations in price among the different brands of medicines, including Tamsulosin 0.4mg capsules, Cefixime 200mg tablets and Amlodipine 5mg tablets.

Furthermore, 26 percent of the health facilities were found to be not storing vaccines in cold chain, which is mandatory, says the report.
There are more than 4,000 government health facilities across the country, including primary health centers, health posts and district hospitals, with about 35,000 health workers including 1,500  doctors, according to the Ministry of Health (MoH). There are 109 government hospitals while the private sector owns 154 hospitals. Besides these,  dozens of private clinics and nursing homes are also operating in the country.
