Health Care Coordinator & Patient Navigator- Sundar Dhoka Sathi Sewa (SDSS)

Sundar Dhoka Sathi Sewa (SDSS) is a registered Non Government Organization in Lalitpur district that aims to provide health services to specific health need persons from the isolated areas of Nepal. SDSS also supports for the economic rehabilitation and physiotherapeutic services to people with disability; creates opportunities for out of school children with disability to access education. SDSS is therefore inviting applications in following three positions for the implementation of its patient navigation programme.

Position : Health Care Coordinator

Required Number : 1
Working Locations : Lalitpur
Position type : Fixed term staff
Reports to : Programme Manager

The Health Care Coordinator will be responsible for implementation of patient navigation, disability rehabilitation and every health initiatives of the organization.

Key responsibilities

  • Coordinate with relevant health institution for care and treatment of patient arrived in SDSS
  • Update record of patient’s health history, care and treatment
  • Develop proposal / concept note for new project, care and treatment of specific need patients
  • Guide and supervise Patient Navigators, Disability Rehabilitation Officer, auxiliary staff
  • Ensure to maintain and update patient file
  • Prepare monthly, half yearly and annual report and share within timeline
  • Prepare success story, visual of patent cases for the purpose of reporting and branding SDSS
  • Prepare project budget and implement detail implementation plan as planned

Requirement and qualification

  • Bachelor in public health, health care management or relevant field
  • Proven experience in health project
  • Knowledge of health care facilities and procedure at various health institution
  • Communication, networking and problem solving skills
  • Competency in Microsoft Applications specifically in MS office
  • Knowledge on disability, inclusiveness

Position : Patient Navigator

Required Number : 1
Working Locations : Lalitpur
Position type : Fixed term staff
Reports to : Health Care Coordinator

The Patient Navigator will be responsible to provide treatment support, rehabilitation, physiotherapy care and services to patients in SDSS.

Key responsibilities

  • Navigate patient along with their caregiver to hospitals and relevant health institution service centre
  • Communicate with patient, caregiver, partner organization for any follow-ups and support for the complete care and treatment of patient
  • Prepare patient history and document for further reference
  • Maintain record of examination, medical consultation, advice and treatment prescribed for the patient
  • Update supervisor about progress on care and treatment of patient on daily basis
  • Plan and request with finance for required budget for patients’ treatment

Requirement and qualification

  • Intermediate degree with CMA or ANM; preference to HA or staff nurse
  • Knowledge of health care facilities and procedure at various health institution
  • Communication, networking and problem solving skills
  • Basic computer skill

Position: Child Rehabilitation Officer / Physiotherapist

Required Number : 1
Working Locations : Lalitpur
Position type : Fixed term staff
Reports to : Health Care Coordinator

The child rehabilitation officer is responsible to provide treatment support, rehabilitation and physiotherapy care to patients in SDSS.

Key responsibilities

  • Provide physiotherapy care to patients, including but not limited to new assessments, general patient treatment and recommendation for the discharge
  • Promote the therapy as a rehabilitation medicine to alleviate adverse effect of disability
  • Communicate with patient, caregiver, partner organization for any follow-ups and support for the complete care and treatment of patient
  • Maintain record of examination, medical consultation, advice and treatment prescribed for the patient
  • Update supervisor about progress on care and treatment of patient on daily basis
  • Support in the financial recordings and reporting as guided by finance person and supervisor

Requirement and qualification

  • Bachelor in Physiotherapy or occupational therapy or equivalent
  • Knowledge on physiotherapeutic care and treatment, psychosocial support and rehabilitation support services
  • Communication, networking and problem solving skills
  • Basic computer skill
  • Knowledge on disability, inclusiveness
