Possible is seeking an entrepreneurial Community Health Program Associate (CHPA) to be a senior leader within the Community Healthcare Team to ensure patient centered clinical care delivery is as close as possible to patient’s home.

The ideal candidate will have the background in health science and experience managing public health interventions. She/He will be able to manage the relationship with the local stakeholders and have ability to engage with the communities. BSc Nursing, BN or BPH with experience managing Public Health Programs.

 Management Structure:

 Reports To: Dolakha District Community Healthcare Director

Direct Reports: Community Health Nurses (CHN)

Big 3 Responsibilities:

  1. Ensure robust implementation of Community Healthcare Programs
  2. Manage team and provide mentorship to Community Healthcare team under her/ his area of responsibility to execute community healthcare programs
  3. Ensure utilization of data and metrics to monitor, evaluate and guide program implementation

Areas of Responsibility:

The Community Health Program Associate should see herself/ himself as a leader of the Community Healthcare Program ensuring individual centered healthcare service delivery in the community is close to patient’s home.

  1. Ensure robust implementation of Community Healthcare Programs
  • Ensure community healthcare program implementation, including the continuous surveillance system, and decentralized care delivery through Community Health Workers (CHW) home visits and group care delivery.
  • Engage with local stakeholders and government partners to coordinate and communicate Community Healthcare Programs and achievements.
  • Coordinate with other teams to ensure all financial, operations, and data systems are in place so that the annual and quarterly objectives are achieved.
  • Ensure adherence to Community Healthcare guidelines, protocols and supervisory schedule while implementing programs.
  • Support Community Health Nurses to ensure all supplies, materials, and training tools are available to plan and execute Community Healthcare delivery at the CHN and Community Health Workers (CHWs) level.
  • Train Community Health Nurses and Community Health Workers to roll-out different programs.
  • Supervise and support CHNs as they facilitate CHW training and meetings.
  • Engage in content generation process for Community Health Workers and Community Health Nurses training.
  1. Manage team and provide mentorship to community healthcare team to execute Community Healthcare programming
  • Supervise, mentor and serve as the direct contact for the Community Health Nurses (CHNs) working within her/ his area of responsibility.
  • Conduct bi-annual performance evaluations for all staff under her/ his direct supervision with comprehensive actionable recommendations provided to staff where needed using Possible’s [email protected] system.
  • Conduct a weekly one-on-one meeting with all direct reports.
  • Be responsible for corrective action and termination of any employees who are not meeting expectations.
  • Be the hiring lead for any direct reports; assist direct reports with their function as hiring lead for staff under them.
  • Ensure compliance to Possible’s communications platform, including asana, Gmail, and Gcal, for effective communication.
  1. Ensure utilization of data and metrics to monitor, evaluate and guide program implementation
  • Ensure utilization of Community Healthcare and Impact developed program tools for care delivery and data collection.
  • CHPA should review the CHP internal monthly data review with their direct reports, and then present findings from their overall coverage area to their manager.
  • Ensure each CHN is reviewing monthly data of their respective CHWLs coverage area in order to identify programmatic and individual gaps and establish and implement program priorities.
  • For targets not met, submit recommendations to your manager on the programmatic action that needs to be taken to improve program performance (program recommendations, HR needs, additional support etc).
  • Assist CHNs and CHWs to improve every aspect of data collection and reporting, including accuracy, internal consistency, completeness, and timeliness

Location: You will spend 100% of your time in Dolakha.

 The above list of responsibilities is not comprehensive, and the Community Healthcare Program Associate may be required to take on additional responsibilities, as determined by the Dolakha District Community Healthcare Director.
